About This Messaging Program
Active SOS operates a closed, business-to-business (B2B) patient alert notification platform exclusively for registered medical practices. This is not a consumer marketing service. No promotional, advertising, or solicitation messages are sent through this program.
When a patient sends a health alert through the Active SOS mobile app, that alert is forwarded as an SMS notification to the registered physician or designated on-call medical staff at the patient's practice.
All SMS recipients are:
- Licensed physicians or authorized medical staff employed by a registered medical practice
- Individually enrolled in the Active SOS Provider Portal by the practice administrator
- Required to actively opt in to SMS notifications during portal onboarding
- Able to opt out at any time by replying STOP
How Opt-In Consent Is Collected
Consent is collected during provider onboarding in the Active SOS Provider Portal. Each physician or staff member who will receive patient alert SMS messages must complete this step individually.
The SMS consent checkbox is presented separately from the required Terms of Service acceptance. It is not pre-selected. The provider must actively check the box to enroll in SMS notifications. Account creation does not require SMS consent — it is a separate, affirmative action.
A provider cannot be enrolled in SMS alert notifications without personally and actively checking the consent checkbox. No SMS messages are sent to a provider until this action is taken.
Representative consent form presented during onboarding:
Full Consent Language Displayed to Recipients
The following exact language is shown to each provider at the point of consent:

Message Frequency & Types

- Urgent SOS patient alerts requiring immediate provider review
- Standard patient health alerts queued for provider review

No marketing, promotional, or third-party messages are ever sent through this program.
